Applying for a Certificate of Conduct (VOG)
To apply for a VOG, you need:

A valid proof of identity. This can be a passport, Dutch ID card or driving licence.
A fully completedapplication form (in Dutch). The organisation that requests the VOG must first fully complete the form and sign it. You cannot file your application until after the organisation requesting it has completed and signed the form. You must submit the original form, not a scan or copy.

There are two ways to apply for a VOG

1, at a city office;

2, You can apply for a VOG online if the organisation requesting it provides that option. The application will be submitted through the Integrity and Screening Agency’s website (Justis).
